Health Information Management / Medical Records

There are times when it is necessary to request your medical records.  According to Massachusetts General Laws, you have the right to view your own record.  You may also receive a copy upon payment.  In order to do this, you must present identification and sign an authorization form.  The review and/or copying is done under the supervision of the Health Information Management Department.

If you are mailing a Release of Information form to request copies of medical records, please remember to include in the address on the envelope, Attn: Health Information Management Department.

 

If you have the need to obtain your birth certificate, the City Hall of the city in which you were born will be able to help you. Also, the National Center for Health Statistics maintains a Where to Write for Vital Records Web site.
